HOPE Mobile

The HOPE Mobile is a mobile medical unit, providing primary and preventive
healthcare services to Logan County residents and surrounding areas.

Individuals under the age of 18 must have a  signed consent form to receive services.

The HOPE Mobile has been providing primary and preventive healthcare services to Logan County residents and surrounding areas, for the past years.

Services include:

· Immunizations

· School/Athletic Physicals

· Pre-employment Physicals

· Chronic Disease Management

· Sick Visits

· Screenings; i.e. glucose, blood pressure, hemoglobin, etc.

· And many more

 

 

The HOPE Mobile is focusing their efforts on improving health conditions and increasing access to quality healthcare services.

Improving the health outcomes of Logan County’s population will be achieved through:

· Decreasing the diabetes rate

· Decreasing the CardioVascular Disease rates

· Decreasing Adult and Low-income youth obesity rates.

In 2010, the HOPE Mobile was awarded a three-year Health Resources and Services Administration grant, through the Office of Rural Health Policy, in order to improve the quality of care and outcomes for patients with chronic illnesses and lessen the risk of harm, outcome improvement and increase the efficacy of our organization.
