Objectives:

-Primary Health Objectives-

* Achieve excellence in care of hypertension patients.
Improve over-all market share by referring 80% of eligible patients to FMC

-Preventive Health Objectives-

* Increase HOPE patients who have a blood pressure of < 140/90 to 78% Baseline: FY2007 75.7% had a B/P reading of >140/90

-Oral Health Objectives-

* Maintain 90% or higher retention rate of sealants.

* Reduce new areas of decay by 10% (baseline is 90%)

* Obtain 70% recall in 6 months
